CLINICAL VALUE OF CARDIOTOCOGRAPHY IN THE ASSESSMENT OF FETAL METABOLIC STATUS IN INTRAUTERINE HYPOXIA

Abstract

This study evaluated the relationship between cardiotocographic findings and the acid–base status of fetal blood in pregnancies with suspected intrauterine hypoxia. The single-center prospective observation included 42 pregnant women at 24–35 weeks of gestation. Cardiotocography scored by the Fisher system, Doppler assessment, and fetal blood analysis after cordocentesis were performed following informed consent. The Fisher score showed moderate correlations with fetal blood pH and extracellular base excess, whereas its association with lactate was not statistically significant. Umbilical artery pulsatility index and cerebroplacental ratio were more closely related to metabolic parameters. The findings indicate that a lower cardiotocographic score accompanies deterioration of fetal acid–base status, but cardiotocography alone cannot quantify the depth of tissue hypoxia. Integrated interpretation of cardiotocographic, Doppler, and laboratory findings provides a more complete clinical assessment of fetal condition and reduces the risk of conclusions based on a single monitoring modality in high-risk pregnancy management and clinical practice.
